The Gevity Health Plan is comprised of four different plans – medication, monitoring, nutrition and exercise – developed by you or experts such as GPs, specialists, nurses, exercise physiologists, physiotherapists or dietitians.
Here’s how it works: If you’ve had a heart attack, as part of your rehabilitation your cardiologist may have referred you to an exercise physiologist, dietician and nurse to closely monitor your recovery. The members of your care network would prescribe a series of exercise, dietary and medication requirements, plus daily monitoring of blood pressure and weight.
Plug these into your Health Plan and Gevity will consolidates your actions onto one simple screen. This lets you focus just on what needs to be done today.
If you choose to share your data with health professionals, family and caregivers, they can monitor your compliance and help you safely rehabilitate. Once you’re back on your feet, Gevity will keep you on track to avoid any reoccurrence.
